Provider openings belong to the participating provider; Calm Waters-operated housing availability is separate and location-specific. A Successful Placement occurs when: Calm Waters first refers or introduces the person; the Provider independently accepts them; move-in arrangements are completed; and the resident physically moves into the Provider’s housing.

An inquiry, application, tour, approval, reservation, or planned move-in alone is not a successful placement.

These agreement and financial terms are business-draft material and are not represented as attorney-approved.

Provider Referral & Placement Agreement — Business Draft

The Provider retains sole responsibility for lawful screening, documentation, pricing, eligibility, availability, and final acceptance. Calm Waters may introduce potential residents but does not guarantee qualification, acceptance, occupancy, revenue, or referral volume.

The standard fee is $300 only after a Successful Placement as defined above, unless a different written agreement is approved. The Provider will report confirmed move-ins resulting from Calm Waters introductions and maintain accurate availability, rates, and requirements.

Submission creates a pending application only. Calm Waters may verify, request clarification, approve, decline, pause, or deactivate participation. Both parties must comply with applicable privacy, fair-housing, and nondiscrimination requirements.
